2013年1月，完成卡塞米羅租借加盟皇家馬德裏，续约2013年6月 ，双方皇馬執行了卡塞米羅的签约買斷選項。2014年，至年卡塞米羅租借至波爾圖 ，皇马租期為一個賽季。官方2015年回歸皇馬一線隊後 ，卡塞卡塞米羅共為皇馬一線隊出戰288場 ，米罗打進30球 ，完成並隨隊獲得4座歐冠冠軍獎杯、续约3次世俱杯冠軍 、2次歐洲超級杯冠軍、2次西甲冠軍 、1次國王杯和2次西班牙超級杯冠軍 。
